Frequently Asked Questions

For any specialty care clinic, you will need a referral from your Primary Care Provider. Once your referral has been received, the clinic will reach out to schedule you based on the type of referral.

If you do not have a primary care provider, you can meet with one of our Primary Care providers to assist:

You can become a new patient at any of our Primary Care clinics by registering in person, over the phone, or by filling out the new patient information form here on the website.

In addition to anything specifically requested, please make sure to bring valid ID and your insurance card to any appointment.
