Patient Portal Support

To better serve you, patient portal communication must be done by e-mail. Should you have a question about patient portal,  you can click the button below to begin an email. Please allow up to three (3) business days for a response.

Sign up for Patient Portal

Click on the button below to download the application form. Please bring the completed form with a valid government issued identification to submit at your next clinic visit or hospital appointment at Admitting, Imaging, Rehabilitation or Medical Records to establish access to your patient portal.

Ask us to Sign up for Patient Portal

Ask the East Hawaii Clinic office staff for an account when you check in or check out. Clinic office staff will verify or collect your email address. Once we have your email address recorded in your patient chart, we will send you an enrollment email. The email you receive will be from easthawaiipatientportal@hhsc.org.  This message will include a link. Click the link to set up your East Hawaii Patient Portal account. The link is only good for 5 days.
